Associate degree in Physical Therapy approved by American Physical Therapy Association required.
Licensed or license eligible in the appropriate jurisdiction required.
In order to work effectively with patients, must have effective interpersonal and communication skills and be able to stand, walk, bend, lift at least 50 pounds, hear, speak, and see.
Prior experience in an outpatient setting preferred.
Meet licensure requirement within one year of employment required.
Two years of full time practice as a licensed physical therapist in a clinical setting or two years as a Career Progression Candidate in the PT Department required.
Five years of clinical experience and have successfully passed an examination of advanced clinical skills and theory in upper extremity rehabilitation preferred.
Graduate of an accredited Physical Therapy curriculum with a Bachelor’s Degree, Master’s Degree, or Doctoral Degree required.
Physical Therapy License in the appropriate jurisdiction required.
Current CPR certification required
National Provider Identification (NPI) required.
